Standard chemotherapy is currently considered best for cancer treatment and most effective. However, when we actually treat the patients, we feel that the response rate is poor and also do not feel that is the best treatment. This is because of a lack of thought of the tumor microenvironment (TME).

The TME, in addition to harboring carcinoma cells, consists of various components that have a major role in influencing the outcome of the malignancy. These can be widely classified into three main groups: cells of hematopoietic origin, cells of mesenchymal origin and non-cellular components (Nature Review). The non-malignant cells have a dynamic and often tumor-promoting function at all stages of carcinogenesis. The TME also plays a role in the cancer recurrence and resistance to chemotherapy. The TME is an important aspect of cancer therapy, and the TME can be changed by molecular target drugs.

We have been using molecular target drugs, especially, thalidomide, celecoxib, valproic acid to change the TME expecting to suppress tumor-promoting function since 2000.
